    As part of the application process, Children Academy may investigate matters such as an applicant's credit standing, credit
    capacity, character, general reputation, and personal characteristics. A criminal background check may be obtained by the
    Company through federal, state, and/or local law enforcement agencies. Further information may be obtained through
    "consumer reports" prepared by a consumer reporting agency or "investigative consumer reports", which are based on
    personal interviews with an applicant's neighbors, friends, associates, or other acquaintances. Upon request, applicants will
    be informed whether a consumer report or investigative consumer report has been requested, the name and address of
    the reporting agency, and, for investigative consumer reports, additional information regarding the nature and results of
    the investigation. If an applicant is denied employment based on information in an investigative consumer report, the Company will so advise the applicant and supply the name and address of the reporting agency making the report.

    Subsequent consumer reports may be requested or utilized, during employment to evaluate suitability for continued employment, reassignment, promotion, transfer or other terms and conditions of employment. If any adverse action is taken because of information contained on a consumer report, the Company will advise the employee of the reason for its adverse action and supply the name and address of the reporting agency making the report.

    By signing this application, I: 1) authorize the Company to contact all of my employment references, and to investigate and
    obtain copies of any records from former employers, educational institutions, other relevant third parties, and any
    government or law enforcement agency; and 2) agree to release to the Company and all affiliate entities, their employees,
    as well as any person or institution that provides the Company with any information about me, from any and all liability
    whatsoever resulting from any such investigation or communication, so long as such information is relevant to my employment. Provide proof of income, including a copy of a recent pay stub or W-2. The Company herein states that it
    shall not use any such information for any improper or unlawful purpose.

    If required by the company, I hereby consent to having a physical and/or medical examination(s) and/or test(s) conducted
    by a physician or other professional of the Company’s choice and understand that an offer of employment may be conditioned upon the results of this examination(s) or test(s). I also agree that should I be employed; I may be required to
    take a company directed physical examination as required by state or local childcare laws/regulations. I consent to having
    the results of such required physical examinations released to the Company.


    All new hires must be fingerprinted and undergo a thorough criminal history screening (CHRI) as well as a thorough child
    abuse history screening (CARI). New Hires may start orientation but may not be scheduled to work directly with children until the result of their background check (CHRI) is received and they are cleared to work. Offer of employment is immediately rescinded if it is discovered that an employee falsified or misrepresented their work history, provided false information, or failed to provide pertinent information whether verbally or in writing during their pre-employment screening process. Offer of employment is also rescinded immediately following hit on their child abuse history screening or failed to receive full clearance from the state of New Jersey.

    New employees who have been cleared for criminal history may begin working in classrooms but should never allow themselves to be left alone without the oversight of staff who has been cleared. You will be informed when your clearance has been received from the state after which you may be allowed to work alone with children. You will be
    terminated if you violate this policy.

    Pursuant to the Manual of Requirement from the Office of Licensing, a criminal record containing the below listed crimes
    disqualifies an individual from employment in a childcare center.

    • An individual shall be permanently disqualified from employment at, or ownership or sponsorship of, a childcare center if the CHRI background check for the individual reveals a record of conviction for any of the following crimes and offenses: In New Jersey, any crime or disorderly persons offense as follows:
      1. A crime against a child, including endangering the welfare of a child and child pornography, pursuant to
        N.J.S.A. 2C:24-4, and child molestation, as set forth in N.J.S.A. 2C:14-1 et seq.
      2. Abuse, abandonment or neglect of a child, pursuant to N.J.S.A. 9:6-3;
      3. Endangering the welfare of an incompetent person, pursuant to N.J.S.A. 2C:24-7
      4. Sexual assault, criminal sexual contact or lewdness, pursuant to N.J.S.A. 2C:14-2 through 14-4
      5. Murder, pursuant to N.J.S.A. 2C:11-3, or manslaughter, pursuant to N.J.S.A. 2C:11-4
      6. Stalking, pursuant to P.L. 1992, c. 209 (N.J.S.A. 2C:12-10)
      7. Kidnapping and related offenses including criminal restraint, false imprisonment, interference with custody,
        criminal coercion, or enticing a child into a motor vehicle, structure or isolated area, pursuant to N.J.S.A.
        2C:13-1 through 13-6
      8. Arson, pursuant to N.J.S.A. 2C:17-1, or causing or risking widespread injury or damage which would constitute
        a crime of the second degree, pursuant to N.J.S.A. 2C:17-2
      9. Any drug related offense committed within the preceding five-year period or
      10. Terroristic threats, pursuant to N.J.S.A. 2C:12-3 or
      11. An attempt or conspiracy to commit any of the crimes or offenses listed
      12. (c)1i through ix above or
    • In any other state or jurisdiction, any conduct which, if committed in New Jersey, would constitute any of the crimes or offenses described in (c)1 above.
